I don’t know when and on which wiki(s) you plan to conduct this survey, but you might want to look into DiscussionTools’ new discussion tool. It’s still in pretty early phase (just got enabled as a beta feature on the first three wikis last week), but it supports visual mode (mini VisualEditor), and preload support is on its way, which will work in both visual mode and wikitext mode.

@Tacsipacsi, thank you! That's right, this tool works, but I'm afraid that pointing at more options (especially not default functionalities which require users to make more steps) would make the instruction less clear. Anyway, we'll discuss this, because the idea sounds fair!

@SGrabarczuk (WMF): thanks for the ping, I am rather curious how a Mobile phone user able to submit their opinion as well? Mine is a grandma kind of query...

• am clear if setting up the form is [ on desktop/laptop];

• and do we mean Mobile users have a unique threshold to join in?
• If not, are we better off to add a clause anywhere that the next step is performed on desktop/laptop but not on mobiles?
• Or mobile users perform by changing to "desktop view", and add a link to change the mode?
